Michael Kubacki, DO
Dr. Kubacki likes to treat every patient like a family member. He holds a DO from the University of Osteopathic Medicine and Health Sciences in Des Moines and completed his residency at The Methodist Hospitals in Gary and Merrillville, Indiana. Dr. Kubacki is a fan of Indiana State University sports and model railroading, and also serves as manager for the Whiting Little League.
Tadeja Agee, MD
Dr. Agee, who sees patients on Mondays, Wednesdays and Thursdays, provides a full array of primary care services, including acute care visits for infants 2 months and older. Dr. Agee earned her bachelor’s degree from University of Washington in Seattle and her MD degree from St. George’s University in Grenada. Besides helping her patients achieve their best self, she loves spending time with her family traveling, exploring new places, and at the lake. Originally from Slovenia, Dr. Agee lives in Fort Wayne.
Susan Schowe, NP
Susan Schowe is a nurse practitioner and health coach with deep experience in the medical field as a registered nurse and nurse practitioner. Prior to joining Activate Healthcare, Susan was an occupational health nurse practitioner for Parkview Health and a family nurse practitioner for Lutheran Medical Group. Her goal is to educate individuals on the benefits of a healthy lifestyle to decrease their risk for chronic disease and illness.
How do I enroll in the NextGen Enterprise Patient Portal (formerly NextGen® Patient Portal) website?
Your practice can enroll you in NextGen Enterprise Patient Portal in one of the following ways:
- Issue a token:Â If your practice initiates the enrollment and provides you a token, use the token to create a NextGen Enterprise Patient Portal account and complete the enrollment.
- Provide a temporary username and password: If your practice provides you a temporary username and password, log on to NextGen Enterprise Patient Portal with the credentials. Type your own username and password to create a NextGen Enterprise Patient Portal account and complete the enrollment.
- Ask you to send a request: If your practice provides you the portal website link or instructs you to enroll by using the NextGen Enterprise Patient Portal link on the practice website, use the link to enroll yourself. After enrollment completion, a submit enrollment request appears that states that request has been sent to the practice and pending for approval. Once the request has been approved, you will receive a confirmation email for the same.
- Send an invitation link to your email address:Â If your practice has invited you to enroll in NextGen Enterprise Patient Portal using the enrollment link sent to your email address, use that link to enroll.
