The Patient Portal provides patients a convenient resource and a home for their Patient First medical records. Once registered, you may use the portal to:
- View Your Medical History: Have full access to your Patient First visit history, on any device.
- View Your Lab Results: Get immediate access to your lab results as soon as they become available.
- Message Us: If you have a question about your recent visit, you may contact us quickly and securely.

To sign up for a Patient Portal Account, visit a Patient First center and speak with a front office employee to obtain an activation code. Once you have a code, navigate to our Patient Portal to register.

Activation codes are invalid after 30 days.
Please ensure that you select a phone number with text messaging capabilities. As an alternative, you can choose the voice option to complete two-factor authentication.

How can I get a referral to a specialist?
In general, you must be evaluated by a Patient First physician before you will be referred to a specialist. Depending on your insurance, you may need to call the Patient First Referral Center at (800) 370-8197 at least two weekdays prior to your appointment so that approval for your visit with the specialist can be obtained from your insurance company. We encourage patients to contact their insurance provider to confirm the referral policy.
How can I get copies of my medical records and other electronic health information?
Consistent with federal and state guidelines (including the 21st Century Cures Act and the HIPAA Privacy Standards), Patient First offers a number of ways for you to access, review and share your medical records and other electronic health information (EHI)
Through this Patient Portal:
You may access, download and transmit your medical records through this Patient Portal by going to “View and/or Transmit Medical Visits and Lab Results.” To use a third party application programming interface (API), go to “Share My Data” and click on “Authorize APIs.”
In person:
You may visit any Patient First center with your current identification to receive a paper copy of(i) your medical records for a single visit or limited number of related visits, or (ii) itemized invoices for individual dates of service. Copies of digital x-rays are also available on disc
